Fellow was built to the standards of regulated industries and organizations where privacy, security, and data governance matter most.

AI Summary by Fellow
  • Every tool on this list will sign a Business Associate Agreement (BAA), but only on a specific plan, and most gate it to Enterprise.

  • Fellow pairs its HIPAA BAA with controls built for PHI review: zero-day retention, transcript and recording redaction, password-protected recaps, and a compliance portal with AI trackers for workspace-wide review.

  • Granola now offers a BAA on Enterprise but covers only its desktop apps; Fathom, Fireflies, Otter and Read AI also gate HIPAA to their top tier.

Protected health information (PHI) shows up in healthcare and healthtech meetings far more often than anyone plans for: a payer call that drifts into a member ID, a clinical ops sync that names a patient, a vendor demo built on real records. The moment an AI meeting notetaker records and stores that conversation, its vendor becomes your business associate under HIPAA. Choosing a HIPAA compliant AI meeting notetaker therefore starts with one question: will the vendor sign a Business Associate Agreement (BAA), and on which plan?

This guide answers that for eight tools, then compares what each one does with PHI once the BAA is signed.

Which AI meeting notetakers will sign a HIPAA BAA?

According to our research, eight AI meeting notetakers will sign a HIPAA BAA in 2026: Fellow, on Enterprise, with transcript redaction, zero-day retention and a compliance portal for workspace-wide review; Otter, on Enterprise, for real-time transcription; Fireflies, on Enterprise with Private Storage switched on, Microsoft Teams Premium or Microsoft 365 Copilot, under Microsoft's enterprise BAA, for organizations already standardized on Microsoft 365; Read AI, on Enterprise+, for intelligence across meetings, email and chat; Fathom, with a signed BAA on its quote-only Enterprise plan, for sales teams already using it; Granola, on Enterprise, for solo users who want a lightweight bot-free notepad on desktop; and Krisp, on Enterprise, for noise cancellation.

In every case above, the free and entry-level plans are not covered, so a clinician who signs up on a personal account to record a care-coordination call is outside the BAA. Confirm the plan, the signed agreement and the devices it covers before any meeting with PHI is recorded.

What makes an AI meeting notetaker HIPAA compliant?

An AI meeting notetaker is HIPAA compliant for your organization only when its vendor has signed a BAA with you for the plan you use, and its safeguards meet the HIPAA Security Rule. No product is compliant out of the box: compliance is a property of the contract and the deployment, not a badge on a pricing page.

HHS has been explicit about what happens when the contract is missing. In its Guidance on HIPAA and Cloud Computing (HHS Office for Civil Rights, published October 6, 2016, last reviewed December 23, 2022), OCR says a covered entity that stores ePHI with a cloud provider without a BAA "is in violation of the HIPAA Rules."

The same guidance treats any cloud service that creates, receives, maintains or transmits ePHI as a business associate, even when it only holds encrypted data. An AI notetaker does all four: it captures audio, transcribes it, stores the transcript and sends summaries to your team and connected tools.

The practical risk is rarely a dramatic breach. It is a care-coordination call recorded on someone's free account, a transcript with a patient's name synced to a CRM, or a summary link forwarded outside the organization. Each one is a disclosure the covered entity has to account for, and each one happened because the tool was chosen for convenience before anyone asked about the BAA.

This is why healthcare and healthtech teams need governance inside the notetaker itself. Fellow signs a BAA on its Enterprise plan and layers data retention options, redaction and a logged compliance portal on top of it.

Use this checklist with any vendor:

  1. Security Rule safeguards. Encryption at rest and in transit, role-based access control, SSO and SCIM so that access ends when employment ends.

  2. Audit controls. Logs of who viewed, shared, redacted or deleted a meeting record, kept in a form you can produce in an HHS OCR investigation or a breach notification review.

  3. Retention you control, applied to the minimum necessary. The ability to delete raw recordings and transcripts on your schedule, keep only the summary you need, and redact PHI that should never have been captured.

  4. No model training on your meeting content. De-identification is not the same as no use. Read the vendor's own policy for your plan, not a sales deck.

A SOC 2 report or a "HIPAA-ready" badge is not a substitute for any of these. Fellow's compliance features support your compliance program; they are not legal or regulatory advice, and your team decides how to configure them.

How do the 8 HIPAA compliant AI meeting notetakers compare?

Fellow is the only tool in this comparison that combines a BAA with zero-day retention, transcript and recording redaction, and a workspace-wide compliance review portal; it is not the cheapest route to a BAA, because its Enterprise plan starts at 10 seats.

The 8 best HIPAA compliant AI meeting notetakers, reviewed

1. Fellow: Best for security and compliance in healthtech and healthcare organizations

Named by The New York Times Wirecutter as its top pick for transcribing and summarizing meetings, Fellow is a secure AI meeting assistant built for organizations that handle PHI, from healthtech product teams to clinical operations and payer partnerships. Meetings are captured, summarized and made searchable with Ask Fellow, while compliance teams set the rules for what is recorded, kept, shared and reviewed.

Certifications: SOC 2 Type II, HIPAA (BAA), GDPR. Fellow never trains AI models on customer meeting data, on any plan.

Key features:

  • Bot and botless recording under one workspace policy; botless capture runs through the Mac or Windows desktop app

  • Compliance portal with Advanced Trackers that flag patient identifiers or health details in non-clinical meetings for workspace-wide review, with every review action logged

  • Zero-day retention for recordings and transcripts, with AI notes kept on their own schedule (Enterprise)

  • Transcript and recording redaction that permanently removes PHI and regenerates the recap (Enterprise)

  • Password-protected recaps, enforceable workspace-wide on Enterprise, plus pause and resume recording and consent capture for auto-recorded meetings

  • In-person recording from the iOS and Android mobile apps.

Pricing: Business is $15 per user/month billed annually. HIPAA and the BAA are on Enterprise, $25 per user/month billed annually, starting at 10 users. See Fellow pricing.

Limitations:

  • SSO, transcript redaction and workspace-wide retention schedules are Enterprise-only.

  • Botless recording on a computer needs the Fellow desktop app installed.

Best for: Healthtech companies and healthcare organizations that need the BAA, retention, redaction and review controls in one tool rather than across several.

Unlike Fathom, which lists its signed BAA only on a quote-only Enterprise tier, or Otter and Fireflies, which gate HIPAA to Enterprise and document neither redaction nor a workspace-wide review portal on their public pages, Fellow pairs the BAA with governance controls a compliance team runs every day.

2. Otter: Best for real-time transcription

Otter is known for fast live transcription across Zoom, Google Meet and Microsoft Teams. It announced HIPAA compliance in July 2025 after an independent assessment.

Certifications: SOC 2 Type II, HIPAA (BAA on Enterprise, arranged through an account manager). Otter's privacy policy allows it to train its own models on de-identified recordings and transcripts.

Key features:

  • Real-time transcription and live notes in Zoom, Google Meet and Microsoft Teams

  • Bot-based capture plus a native device-level recording option

  • Transcription in 6 languages (English, Spanish, French, German, Japanese, Chinese Simplified)

  • Admin controls and data retention policies on Enterprise

Pricing: HIPAA and the BAA are on Enterprise only, which is quote-only.

Limitations:

  • HIPAA is Enterprise-only and sales-led; lower plans cannot hold PHI.

  • Redaction and a workspace-wide compliance review portal are not documented on Otter's public pages.

  • Otter is a defendant in In re Otter.AI Privacy Litigation (N.D. Cal., No. 5:25-cv-06911). On August 13, 2026, the court granted Otter's motion to dismiss in part and denied it in part: the federal wiretap (ECPA), California privacy (CIPA), Illinois biometric (BIPA), UCL and unjust enrichment claims proceed, while the CFAA, CDAFA and Washington Privacy Act claims were dismissed, many with leave to amend. Otter answered the complaint on September 17, 2026. No court has found Otter liable.

Best for: Teams whose first need is fast, accurate live transcription and who can budget for Otter Enterprise to get BAA coverage.

3. Fireflies: Best for teams with specific CRM integration requirements

Fireflies suits organizations that record a large number of meetings across sales, support and internal syncs and want channels, global search and conversation intelligence across them.

Certifications: SOC 2 Type II, ISO 42001, GDPR, HIPAA (BAA on Enterprise with Private Storage). Fireflies states customer data is never used for AI training.

Key features:

  • Channels, global search and AskFred across a team's meetings

  • Conversation intelligence, topic trackers and team analytics (Business and above)

  • Bot-free capture through the desktop app and Chrome extension

  • Rules engine, audit logs (via API), custom data retention and Private Storage on Enterprise

Pricing: Free plan available. Pro is $10 and Business is $19 per seat/month billed annually ($18 and $29 monthly). HIPAA is on Enterprise, $39 per seat/month, annual billing only.

Limitations:

  • HIPAA is active only while three conditions hold: Enterprise plan, Private Storage enabled and a signed BAA, completed by admins in the product. Removing any one switches HIPAA off.

  • Enterprise cannot be billed monthly.

  • Zero-day deletion of recordings and transcript redaction are not documented on Fireflies' public pages.

Best for: Organizations with heavy meeting volume across many teams that want specific CRM integrations.

4. Microsoft Teams Premium / Microsoft 365 Copilot: Best for organizations standardized on Microsoft 365

For teams already running Microsoft 365, Teams intelligent recap produces AI meeting notes inside Teams. Coverage runs through Microsoft's BAA for in-scope commercial services rather than a new vendor contract.

Certifications: HIPAA through Microsoft's BAA for in-scope commercial Microsoft 365 services. Confirm that each Teams and Copilot feature you use is listed as in scope for your tenant.

Key features:

  • Intelligent recap with AI notes, suggested tasks, speaker markers and chapters

  • Audio recap (Copilot, public preview) and video recap for recorded meetings

  • Recap without saving the meeting transcript for scheduled meetings (Copilot)

  • Recap sharing with external attendees through Outlook

Pricing: Requires a commercial Microsoft 365 plan plus a Teams Premium or Microsoft 365 Copilot license per user. Copilot Business is a newer add-on for organizations of up to 300 users.

Limitations:

  • Every user who needs recap needs an add-on license, and audio recap needs Copilot specifically.

  • BAA coverage depends on Microsoft's in-scope services list, so IT has to confirm it feature by feature.

  • A recap-level redaction or compliance review queue is not documented in the Teams docs reviewed.

Best for: Healthcare IT organizations that want to stay inside their existing Microsoft 365 contract and procurement relationship.

5. Read AI: Best for cross-channel intelligence (meetings, email, chat)

Read AI extends beyond meetings into email and messaging, with Ask Read search and summaries across all three. It suits ops and revenue teams that want one assistant across channels.

Certifications: SOC 2 Type II, GDPR, HIPAA (Enterprise+ only).

Key features:

  • Ask Read search across meetings, email and messaging

  • Meeting reports with summaries, topics, action items and key questions

  • Desktop and mobile apps, including in-person capture

  • Data retention policy, SAML, SCIM and domain capture on Enterprise+

Pricing: Free plan available (5 meetings per month). Pro is $15, Enterprise $22.50 and Enterprise+ $29.75 per user/month billed annually ($19.75, $29.75 and $39.75 monthly). HIPAA is on Enterprise+, which shows a 5-license minimum.

Limitations:

  • HIPAA is available only on the top tier.

  • Retention deletes whole meeting reports after a set number of days; a separate zero-day setting for raw recordings is not documented.

  • Redaction and a workspace-wide compliance review portal are not documented on Read AI's public pages.

Best for: Ops and revenue teams that want meeting, email and chat intelligence in one place.

6. Fathom: Best for sales teams already on Fathom

Fathom built its following on a generous free plan and sales features on its Business tier. Superhuman announced it had acquired Fathom on September 14, 2026; Fathom says the product and login are unchanged for now.

Certifications: SOC 2 Type II, GDPR, HIPAA (signed BAA listed on Enterprise). Fathom's policy allows training its own models on de-identified meeting content, with an opt-out.

Key features:

  • Unlimited recordings, transcription and instant AI summaries on the free plan

  • CRM field sync, deal view and coaching scorecards (Business)

  • Bot-free capture, in beta on Mac

  • SSO, SCIM, custom data retention and organization-wide security controls on Enterprise

Pricing: Free plan available. Premium is $16, Team $15 and Business $25 per user/month billed annually ($20, $19 and $34 monthly), with a 2-user minimum on Team and Business. Enterprise is quote-only.

Limitations:

  • Fathom's own pages describe HIPAA two ways: the help center points to a blanket BAA linked from its terms and offers a signed BAA on request, while the pricing page lists "HIPAA: signed BAA" only on Enterprise, a quote-only tier added in July 2026. Ask Fathom in writing which agreement applies to your plan.

  • Bot-free capture is a Mac beta.

  • How Superhuman will handle Fathom data and policies has not been announced.

Best for: Sales teams already using Fathom that want to keep CRM workflows in place.

7. Granola: Best lightweight bot-free option for solo users

Granola is a notepad-style tool that captures audio on your device without a bot joining the call and deletes the audio once the transcript is created. It works best for individuals who want their own notes rather than a shared team record.

Certifications: SOC 2 Type II, HIPAA (BAA on Enterprise, desktop and laptop apps only). On Basic and Business, Granola trains its models on de-identified data by default; Enterprise turns that off organization-wide.

Key features:

  • Bot-free capture on Mac and Windows, with audio deleted after transcription

  • A personal notepad that AI enhances with the transcript

  • iOS and Android apps for in-person notes

  • SSO, SCIM and organization-wide training opt-out on Enterprise

Pricing: HIPAA and the BAA are on Enterprise, from $35 per user/month.

Limitations:

  • The BAA covers only the desktop and laptop apps, so in-person notes taken on a phone are outside it.

  • Each person keeps their own notes, so there is no single shared record per meeting.

  • Model training is on by default below Enterprise.

Best for: Individual users who want lightweight, bot-free notes on their own computer.

8. Krisp: Best for noise cancellation

Krisp is best known for AI noise cancellation, and it now sells a full AI Meeting Assistant with bot-free transcription, recording, AI notes, action items and CRM sync.

Certifications: SOC 2 Type II, GDPR, HIPAA, PCI DSS. Krisp states that meeting data is never used to train its models.

Key features:

  • Noise cancellation and accent conversion on every plan

  • Bot-free AI notetaker for online and in-person meetings

  • On-device private transcription and recordings (Enterprise)

  • HubSpot, Slack and MCP integrations, plus Salesforce on Advanced

Pricing: 7-day free trial. Core is $8 and Advanced $15 per user/month billed annually ($16 and $30 monthly). Enterprise is custom.

Limitations:

  • Krisp's pages disagree on which plan qualifies: pricing lists HIPAA on Enterprise and BAA signing on Enterprise, or on Advanced for teams of 100 or more seats, while the security page says BAAs are available only on a "Business" tier that no longer appears on the pricing page.

  • AI Meeting Assistant data is stored only in the United States.

  • Redaction and a workspace-wide compliance review portal are not documented on Krisp's public pages.

Best for: Teams on noisy calls or shared spaces that want clean audio and AI notes from one app.

Conclusion

Every tool on this list will sign a BAA, so the real decision is what happens to PHI after it is signed: how long recordings live, whether PHI can be redacted, and whether your compliance team can review meetings without relying on what individuals choose to share. Fellow answers all three inside one AI meeting assistant, and its SOC 2 Type II report and HIPAA documentation are available for review in the Fellow trust center.
